DESCRIPTION:
If you get a call while listening to music through your headphones (BT in my case) you don’t hear the ringtone through the headphones.

Listen to music
Get a phone call
EXPECTED RESULT:
Listen to the ringtone sound on the headphones (and from the speakers to cover all bases)
ACTUAL RESULT:
No sound on your headphones

This is an old bug. I remember this from when i was running J1 as a daily.
It is strange, but when I tried it with my headphones “Urbanears Plattan II BT”, connected to Xperia 10 II with SFOS 4.2, I heard ringing in headphones. I’m pretty sure that it was working even in my car (Škoda Rapid 2016).
It is little bit weird that sound was different from default Sailfish ringtone that I’m using. I’m not expert on A2DP and HSP/HFP profiles, but it is possible that ringtone is played by headphones itself and phone just signaling that someone is calling?
To dig this issue deeper, I would suggest you to try different headphones and try to modify flags in “ringtone” (?) group in /etc/pulse/xpolicy.conf file…
